Fairfax Community Health Care Network

What: The Fairfax County Health and Human Services System is a network of county agencies and community partners that supports the well-being of low-income and/or uninsured Fairfax County residents. They operate full-time clinics and partner with other community providers to offer comprehensive medical care and health services. Services offered include: Primary care, dental care, behavioral health services, pharmacy services, and mental health care management.

Costs: Sliding scale fees provide primary care for $15-$25 depending on family size and income.

Who: Fairfax County residents who meet insurance and income eligibility. Uninsured, unable to receive health insurance, family income at or below 200% FPL, Fairfax County residents are eligible for sliding scale fees.

How to access: Visit the CHCN webpage or call the clinic of your choice directly to start the enrollment process. If you meet eligibility requirements, you will be sent information and asked to come in for an interview. Some clinics offer walk-in women’s health services one day a week; most other services require an appointment. Bring or be prepared to provide proof of residency, proof of income, a photo ID, relevant medical records, and insurance information (if applicable).
