ADAMS Compassionate Healthcare Network

What: ADAMS Compassionate Healthcare Network is designed to provide medical services to low-income and uninsured individuals. They offer primary care services, diagnostic and laboratory services, pharmacy enrollment and medication support, in-house subspecialty services, and partner referrals. 

When: Monday–Wednesday, 9:00 AM-3:00 PM.  Saturday – 9:00 AM-12:00 PM. 

Where: 4437 Brookfield Corporate Drive, Suite 109, Chantilly, VA 20151. 

Costs: All primary care services are free. 

Who: If you are uninsured and either your family's income is within 300% of the FPL, or you receive benefits such as SNAP, TANF, Section 8 housing, unemployment benefits, Social Security benefits, or the School Free Lunch Program, you qualify for free consultations and discounted services.

How to access: To request an appointment, call 703-542-3366. If you wish to register as a patient without an appointment, first, complete the registration packet (Registration Packet). You will need to provide a photo ID and proof of income. You can email, fax, mail, or bring in all of the necessary documents to the clinic.
